Family Health

Family doctors care for you and your family throughout the different stages of your lives, and can provide all family members, from older adults to children, with comprehensive care, focusing wellness and quality of life, as well as your physical or mental health concerns. Our family doctor becomes your first point of contact when you need medical care, advice and referrals to specialists, our wellness programs, and assistance services.

Express Care

BMS Family Health and Wellness Centers has opened the new BMS Dumont Express Care Center (located at 407 Dumont Ave; Brooklyn, NY 11212) to provide care on an unscheduled walk-in basis to individuals with acute, non-emergency conditions. The Center offers six exam rooms, as well as lab services. 

    This will be the first Express Care center operated by a stand-alone Federally Qualified Health Center (FQHC) in North and Central Brooklyn. 

    Patients who do not have a medical home or usual source of care will be provided with information and assistance in selecting a primary care provider that meets their needs (location, specialty services, hours, language access). 

 

Come to Express Care for:

  • Fever, Cold, Sore Throat

  • Minor Cuts and Bruises

  • Sprains, Strains and Fractures

  • Asthma and Allergies

  • Women's healthcare including UTI and STD

  • Pediatrics (6 months and older)

In addition to care, the center will provide: 

  • Care coordination 

  • Medical lab and testing (phlebotomy) 

  • Pharmacy (coming soon)

  • Diagnostic imaging (coming soon)

Hours: Monday-Friday 8am-8pm, Saturday, Sunday 9am-5pm

Adolescent Health

BMS strives to empower children and young people by bringing tools to take command of their health. At our seven facilities, BMS offers healthcare and educative services to the children and youth of Brownsville, East New York, and the surrounding neighborhoods. Our BMS professionals are equipped with the cultural and community knowledge to serve children and adolescents with kindness and respect.  

The services we offer include:

Women's, Uterus Owners' &
Birthing Parents’ Health

BMS clinicians and Obstetrics and Gynecology (OB/GYN) provide patients with the highest quality health care for women, vulva/uterus owners, and birthing parents, by maintaining the highest standards of clinical practice and stressing the importance of continuing patient education to ensure their understanding and participation in their medical care plans. 

Our services include:

  • Prenatal and post-partum care and referrals

  • Text4Baby (health information to new parents via cell phones starting in pregnancy and extending through their babies’ first year)

Older Adult Health

We support patients 60 and older in the prevention of health complications and hospitalizations with user friendly and easy to navigate resources. 

 

Our team screens and follows up to make sure our
patients receive: 

  • Education on how to appropriately care for chronic conditions such as diabetes and prescriptions management

  • Access to therapy services

  • Adequate transportation to attend medical appointments

  • External assistance services such affordable housing and home care 

  • Direct care and rehabilitation services 

 

Our compassionate team encourages and teaches clients to help themselves within their own capabilities in performing daily life activities and guides them in their journey to living a healthier life. 

Locations

BMS’ Pediatrics serves patients from newborn to young adults. Our services range from routine health screenings for childhood diseases and immunizations to adolescent services.

About our BMS lead Pediatric clinician: 

Claude E. Offord has been with BMS for over 40 years, becoming one of the most trusted and well-known service providers among our BMS community. Dr. Offord received his MD from Rutgers Medical School in 1981 and completed pediatric residency at Children’s Hospital of New Jersey/UMDNJ.

Onsite Pharmacy

We constantly work with the government and stakeholders to bring affordable medications through BMS’ on-site pharmacy, easing access to the right care for our patients and their families. Our on-site pharmacy is located at Main at Rockaway Avenue. BMS implements the federal 340B Drug Pricing Program allowing low-income and uninsured patients to buy prescription drugs at a lower price.
